Basically, the Halo seems to be ineffective if the Wielder is fully convinced that a certain action or opinion is the “right thing to do/believe”, which means a deranged cereal killer could get away with anything he would fully justify as a ‘good thing’, for example. Or the same for another type of person.
I theorize that the Halo works on the presence of any ‘doubt in one’s heart and mind’ and thus decides morality based on the wielding individual’s honest values.
This is why you need things like empathic connection or the penance stare to make them feel the suffering of others. Without going into the various psychological conditions that could be involved other than simply being convinced their actions are for the greater good as others have pointed out, a technique that relies on a subjective concept like good and evil may not work so well

We also saw this kind of behaviour in Gyrognome, for whom the thought of not dickpunching people was enough to constitute a good deed.
